Guided Care Program

A person-centered approach when caring for older adults.

​What is Guided Care? As we age, families, caregivers, spouses and other loved ones may find themselves in need of help navigating through all the complexities of senior care.

 

Where to start? Who do I call? What type of care does my family need? What is the cost and who pays for this? Is there financial assistance?

​

Aging Matters' Guided Care Program can help you through the entire process, with a supportive person-centered approach.

How does it work?

A private consultation with you or your
family- A comprehensive care needs
assessment is completed. Assistance will be
provided with specific care needs. This may
involve, facility placement, move
coordination. Transition follow up is also
important to ensure your loved one is,
happy and any additional services needed.
Connecting to community services,
resources and care providers is also an
important part of the Guided Care Program.
Assistance with navigating through
Medicare, Medicaid and the Division of
Assets, long-term care insurance policy
review and family mediation/coaching are
all services we can provide through Guided
Care.
